Carefully complete this form with the necessary information, including the first and last
name of the original license holder and the first and last name (spelled EXACTLY as you would like it to appear
in the signed note) of the new license holder. If your providers currently use the MediNotes Report module,
and you would like this functionality to be made available to nurse and staff licenses as well, please check
the appropriate box.

Clinical Staff
By default, clinical staff will not have access to MediNotes Reports. If your clinic
has previously purchased MediNotes Reports, use the checkbox for the users that you would like to have access.
Note: Registered name MUST have a first and last name. Please use capital letters.
Nurse with Signing License
By default, nurses will not have access to MediNotes Reports. If your clinic has previously purchased MediNotes Reports, use
the checkbox for the users that you would like to have access.
